The staff and doctors at Woodland Veterinary Hospital and Pet Hotel would like to take a moment to thank you for choosing to board your pet with us. In an effort to make the check-in process easier, please complete this e-form prior to your pet's stay. Also, if your pet is vaccinated elsewhere, please bring in a current copy of your pet's vaccine records. Dogs must be current on Influenza, Bordetella, DHPP, and Rabies. Cats must be current on FVRCP and Rabies. We recommend pets receive these vaccines at least 2 weeks prior to boarding for full immunity.

PLEASE READ THIS CONTRACT CAREFULLY

This contract contains the sole and exclusive terms and conditions under which you, the undersigned, have entrusted your pet to board with Woodland Veterinary Hospital, Inc., a California Corporation (hereinafter the boarding sub-entity to be designated as the “Woodland Pet Hotel”). By signing this agreement and leaving your pet with Woodland Pet Hotel, you certify to the accuracy of all information provided in writing or orally about your pet.

I, the undersigned, hereby represent and warrant to Woodland Pet Hotel the following:

I certify that my pet is current on all vaccines and affirm that I am responsible for providing proof thereof. Vaccines required for dogs are Rabies, DHPP, Bordetella, and Influenza.

I, the undersigned, represent and warrant to the Woodland Pet Hotel, that I am the pet’s owner or authorized agent of the owner and have full authority to enter into this contract; that my pet is in good health; to the best of my knowledge has not had or been exposed to communicable diseases within the last 30 days prior to entrusting her/him with the Pet Hotel; or he/she has had such a disease but has been symptom-free for at least seven days and/or has a veterinarian’s clearance, and is not known to be aggressive to other animals or humans. I further warrant and represent that my pet is not prone to any particular types of sickness or injuries, except as described on the PET HOTEL CHECK-IN sheet.

SERVICES AND CARE PROVIDED TO BOARDING PETS

MEDICAL CARE IS AVAILABLE BY THE WOODLAND VETERINARY HOSPITAL STAFF BUT IS NOT INCLUDED IN THE NORMAL BOARDING FEES OR SERVICE PROVIDED BY THE PET HOTEL STAFF. DUE AND REASONABLE CARE IS MADE TO MAINTAIN THE OPTIMAL HEALTH AND COMFORT OF YOUR PET, BUT ILLNESS OR INJURY CAN OCCUR.

Please review #2 below and indicate your desire in the event your pet becomes ill and we cannot reach you.

Option 3. Woodland Veterinary Hospital does not maintain staffing on the premises during night time hours. In the event that specialized care, including emergency surgery, or overnight ICU care is deemed necessary by the medical staff at Woodland Veterinary Hospital to effectively treat a serious medical condition, and I cannot be reached by phone for approval, I authorize transportation of my pet to the Veterinary Teaching Hospital at UC Davis, and I assume responsibility for all associated fees and costs.*
If “accept” is chosen, we ask that you take responsibility for establishing an account at the VMTH in the event you are unreachable by phone. Woodland Veterinary Hospital can arrange for transport, but not financing.

ASSUMPTION OF RISK

Boarding in a kennel and in close proximity to other animals can be stressful for pets, can potentially allow exposure to communicable diseases, and can increase the risk of illness or injury. The Pet Hotel and Veterinary Hospital staff exercises due and reasonable care to minimize stress and exposure to harmful agents. Disinfection protocols and mandatory vaccination do not guarantee protection, nor do they protect against all communicable diseases that may affect a pet. Pets in stressful situations such as a kennel are also at increased risk to contract illnesses without exposure to an infectious agent on these premises. As the undersigned, I acknowledge and agree to assume all risk of illness, disease, injury, harm or otherwise to my pet during his/her stay at Woodland Pet Hotel.

I, the undersigned, expressly assume the risk of these and other conditions and diseases and waive any right to bring a claim against Woodland Pet Hotel/Veterinary Hospital, its employees, officers, principals, agents, invitees, guests, insurers and their successors in interest.

I, the undersigned, am hereby warned that if group playtime is selected as part of my pet’s stay, that there is a degree of risk to mix animals who may have no history mixing with others or with these particular animals. Woodland Pet Hotel attempts to minimize this risk by screening pets that may be prone to act aggressively or cause injury, but cannot guarantee adverse consequences. These risks include, but are not limited to, contracting diseases, wounds, bites, other injuries or conditions, etc.

INDEMNIFICATION OF WOODLAND PET HOTEL

I hereby assume all responsibility for my pet while he/she is in the custody of Woodland Pet Hotel, indemnify and hold Woodland Pet Hotel, its employees, officers, principals, agents, invitees, guests, insurers and their successors in interest harmless including the cost of defense including attorney’s fees for the breach of any of the above warranties and representations by me, any damages or injuries caused by my pet to any other person, animal, and/or property; and/or any injury to my pet from any cause whatsoever, except for the gross negligence or willful conduct of Woodland Pet Hotel/Woodland Veterinary Hospital. I further agree to be solely responsible for any and all acts of behavior of said pet while in the care of the Pet Hotel/Veterinary Hospital, including costs for injury to staff or other animals, or damage to facilities caused by the pet.

MISCELLANEOUS PROVISIONS

  1. Parole Evidence Rule – The above constitutes the entire contract between these parties and neither party is relying on any written or oral statements not contained herein. This contract may be modified in writing signed by both parties only and by no other means.
  2. Jurisdiction and Venue – This contract is entered into in the County of Yolo, State of California and shall be enforced only in said county and state and under the laws of the state of California.
  3. Limitation on Damages – In any dispute arising under this Contract claiming an injury or death to any animal or person there shall be no punitive damages awarded, no damages for emotional distress, and any damages for the injury or loss of an animal’s life shall be calculated on the fair market utilitarian value of the animal in keeping with California law as of the date of the making of this contract.
  4. Abandonment – Any animal left at Woodland Pet Hotel for longer than 14 days past the scheduled pick-up date is grounds for determining that you have abandoned your pet. California law has specific laws protecting veterinarians for their fees and costs incurred for abandoned animals. If you are not going to be able to keep your scheduled pick-up date and time, you must make arrangements with the Woodland Vet Hotel, including payment, prior to the scheduled pick-up date.
Execution of Contract – I, the undersigned, hereby declare that I have read this contract carefully and understand by signing this contract I am personally responsible for all liability under this contract for the pet and pet owner. Personal Belongings

We provide plenty of toys, blankets, and bowls here for your pet's comfort. We prefer not to take any personal items into our boarding facility, so we do ask that you please keep personal items of sentimental value at home for your pets to enjoy upon their return. Collars and leashes will be given back to you at check-in. Clean and comfortable bedding is provided during your pet's stay. We encourage you to leave your pet's bedding at home, as special blankets may become soiled or torn up by your pet. We cannot accept responsibility for lost or damaged items left during the stay due to the unpredictable nature of pets while away from home. We will not replace or reimburse for any lost or damaged items. Please note: it is illegal for veterinarians to administer or store any cannabis or cannabinoid products.

Medical Needs

There is an additional fee for administering medications, vitamins, or supplements, even if they are already mixed into your pet’s food. Please note that per federal law, we cannot legally administer or store any cannabis or cannabinoid products.

Note to Our Clients

Owners will be responsible for notifying staff of any changes on this form.

We do our best to not misplace personal items, but cannot be responsible for lost items. Please do not bring items of sentimental value.

If you choose to bring food from home, bring only the amount necessary for the boarding visit. Make sure food is packaged in an airtight container or bag.

If you are bringing a puppy or kitten for boarding, please bring their diet from home.
